CAMC Foundation Nursing / Allied Health / Medical Student Scholarship

Criteria

Thank you for your interest in a CAMC Foundation Nursing/Allied Health/Medical Student Scholarship. Applications will be accepted from May 1st through June 1st 2010 to be considered for the academic semesters: Fall 2010 and Spring 2011. Once an application is submitted, all supporting documents must be presented in a portfolio and received in the Employment Center of Charleston Area Medical Center by June 1, 2010 at 4:30 p.m. The Employment Center is located at 511 Brooks Street - Charleston, WV (304) 388-7458.

Failure to submit all required documents will result in an incomplete application and will not be considered for review. We will not be responsible for documents submitted in any other format.
Portfolios must contain the following information.

Cover letter to include information about you, your educational aspirations and scholarship need.
Letter of acceptance from College or University (must be currently enrolled in a Nursing/Allied Health or Medical Student program)
Copy of current semester grades - official transcript not required (must have a 2.5 GPA to be considered)
Three reference forms completed by a college instructor and or employer
A copy of your most recent tax return for yourself or for which you have been listed as a dependent.
If you indicate that you have completed community service, please complete the Community Service Verification Form
